Acute kidney injury after TAVR: Key risk factors every cardiologist should know

New research is bringing into sharp focus the critical danger posed by acute kidney injury (AKI) in patients undergoing transcatheter aortic valve replacement. This post-procedural complication is now strongly linked to a significantly increased risk of both short- and long-term mortality, making its prevention a top priority for heart teams.

Cardiologists are advised to closely monitor several key patient factors that elevate AKI risk. A history of pre-existing chronic kidney disease remains the most potent predictor. The procedure itself also contributes; prolonged operative time, the use of large volumes of contrast dye for imaging, and certain procedural complexities can all impair kidney function.

Patient-specific conditions further compound the risk. Those presenting with anemia, low blood pressure, or diabetes mellitus are particularly vulnerable. The studies underscore that a multifaceted assessment is essential. By identifying high-risk patients before the procedure, clinicians can implement protective strategies, such as meticulous fluid management and minimizing contrast exposure, to improve overall outcomes and survival rates following this life-saving heart procedure.
